Rising demand for care services, combined with restricted or reduced funding, is increasing pressures on health and social care services. The Department of Health, NHS England and Local Governments are attempting to meet these pressures in various ways, one of which is to improve integration of health and social care services at a local level. As part of this process the team at Carefound Home Care are continually seeking to enhance the expertise across their elderly home care teams in Yorkshire and Cheshire, and in doing so improve their ability to work in partnership with local health teams.

Jan Wrigglesworth is a Nurse Manager for Leeds Community Healthcare NHS Trust and recently took the time to meet with members of the Carefound Home Care team in Harrogate and deliver a specialist training session covering continence care.

The session was a huge success and the carers were delighted to have been able to tap into Jan’s specialist knowledge. At the end of the afternoon Jan Wrigglesworth said:

“I really enjoy this part of my role – being in a position to integrate health and social care and provide information and training to carers in the community.”

Lorna Dawber, Home Care Manager at Carefound Home Care Harrogate, said:

“At Carefound Home Care we are continually striving to provide our carers with the best training opportunities, including how to care for our elderly clients with specialist needs. We are very grateful for Jan Wrigglesworth having taken the time to carry out this training with us and look forward to continuing to develop our relationship with Leeds Community Healthcare NHS Trust.”

Lorna added:

“There are still many barriers to integrated services across health and social care. We believe that building links, encouraging information sharing and joint working is going to be key to improving integration and enabling families to receive the most cost effective care, when and where they need it.”
