Working in partnership with local health professionals

At Carefound Home Care we offer families a highly personalised home care service which enables elderly people to continue to live in their own homes with less reliance on primary care, secondary care or residential care services. We do this in partnership with local health professionals including GPs, community nursing teams, hospital teams, social services teams, occupational therapists, physiotherapists, hospice teams and many more.

Our care is delivered by our professional, trained, vetted staff all of whom are fully employed and supported 24/7 by our care management team. Services can be delivered on an hourly care or full-time live-in care basis, and may include anything from basic domestic support, companionship, personal care, medication help, through to managing complex conditions such as dementia or Parkinson’s.

We are widely regarded as the leading provider of elderly home care in Cheshire, Nottinghamshire and Yorkshire. Our Yorkshire office based in Harrogate is rated “Outstanding” by CQC putting it amongst the top 2% in the UK and is one of the only care providers in the country to have achieved this prestigious rating for a second time. Our Cheshire office based in Wilmslow is also rated “Outstanding” by the CQC and in 2018 become one of the only home care providers in the country to be judged as Outstanding in all five areas – safe, effective, caring, responsive and well-led. Our Nottinghamshire office based in West Bridgford opened in Summer 2020 having been approved by the CQC.

Importantly, and unlike many other social care providers, we have a genuine focus on achieving improved health and social outcomes for our clients – something we constantly monitor. This enables us to develop effective partnership working with local health professionals and reduce pressure on primary and secondary care services:
