This marks the latest edition of our annual review at Carefound Home Care which examines how our service is impacting on people’s lives, what we are doing well, and how we can keep improving.
Since we published our previous review last spring, challenges for care services have grown as social care reform has been delayed and pressures from increased employment costs have come into force. Nevertheless, the desire amongst older people to remain at home and stay physically and mentally active has only increased.
At Carefound Home Care, we remain dedicated to improving the lives of our clients and their families by providing outstanding care and support in the comfort of their own homes, whatever their needs.
Highlights from our client health outcomes
Preventing falls
25% fewer falls experienced by our clients following introduction of our falls prevention strategy
Falls are the largest cause of emergency hospital admissions amongst older people. We introduced our falls prevention strategy in partnership with the NHS in 2021 and this has continued to drive significant reductions in falls amongst our clients. Our approach includes regular falls risk assessments with interventions included in care plans, working with occupational therapists and other health professionals and providing advice on how to avoid falls or regain confidence, monthly falls huddles, and additional falls prevention training for our care professionals.
Identifying and treating infections
95% of our clients are prescribed antibiotics within 48 hours of an infection being identified using our soft sign checks
Spotting signs of infection early and enabling prompt treatment is crucial for preventing complications from infections such as urinary tract infections (UTIs) and chest infections. Our care professionals are trained to spot signs of deterioration in our clients, and they complete soft sign checks using our care management app at every visit. As a result, our clients are receiving timely treatment, making them less likely to be admitted to hospital.
Managing medicines safely
39% reduction in medication incidents experienced by our clients over the last 3 years
We have continued to invest in ensuring our care professionals are trained, competent and confident in how to administer and manage medications safely. We also use our electronic eMAR system in our care management app to provide people with accurate real-time information, so that medicines are administered as prescribed. This has enabled us to consistently reduce the number of medication incidents experienced by our clients every year.
Improving end of life care
65% of our clients who passed away were supported to do so in the comfort of their own home
Around 70% of people say they would prefer to die in their own home, but only 23% of deaths in those aged 85 years or older actually happen at home. Our end-of-life care policy has been developed in partnership with local community health and palliative care practitioners. This ensures that our clients have a personalised end-of-life care plan where appropriate, and our care professionals are trained so that they have the professional and emotional competency to deliver end-of-life care. This enabled almost two-thirds of our clients who passed away in 2024 to do so in the comfort of their own home, rather than a clinical setting.
Reducing hospital admissions
12% reduction in hospital admissions experienced by our clients over the last 12 months
Continued improvements in our clients’ outcomes resulted in a further reduction in hospitalisations during 2024. Furthermore, only 7% of our clients experienced a readmission to hospital within 28 days, compared to 18% of people aged 75 years or over in England. Once again, this shows how our clients benefit from our proactive, highly personalised approach to home care, underpinned by digital technology and delivered by a team of trained and locally supported care professionals.
Feedback from our client surveys
100% of our clients and their families said they were satisfied with their care which is exceptional. 92% also said the service improved their quality of life, and 98% would recommend Carefound Home Care.
Our investment in the training, competency and expertise of our care professionals stood out with 94% of clients saying their care professionals were well trained.
Our home care service is highly personalised with carers carefully matched to people’s needs. This was clearly recognised with 98% of clients saying they felt comfortable and safe with their care professionals, 96% agreeing that their care professional was well matched to their needs, and 92% saying they were happy with the number of care professionals they see.

An impressive 90% of staff said they are satisfied with their job, 94% are proud to work for Carefound Home Care, and 90% understand why we’re different from other care companies.

The support our local care management teams provide was recognised with 94% of staff saying management are approachable and treat them with respect, and 92% saying there is a good level of communication across our team.
91% of care professionals also said they are given appropriate information about their client, and 95% feel they are given enough time to deliver care. This shows that our technology and local teams help our staff feel better informed about their clients and supported to make a genuine difference to their lives.
